WC-ARES Medical Cards

Instructions for WC-ARES Medical Cards

These are simple Word documents. They are formed on a business card type template. They are also set up so you can do more than one if you wish.

Step 1: Print out the cover page, with the WC-ARES logo on it. The using a business card as a guide, cut out one row, all the way across making sure to get all the Text in the card. DO NOT CUT for length, it is designed to fold over so that both sides have the Logo and the instructions on it.

Step 2: Pull up and download the inside sheet. These are just simple text boxes. The information asked for is just my thoughts based on what is required for an EMS person to know if you are unable to answertheir questions. You are welcome to change it to list whatever information you think is important. No one will see this information except you and medical personnel.

Step 3: Print out the inside page. Cut it to match the size of a business card. Either fold and place or tape to the back side of the cover cards. It is designed so that when medical personnel cut around the edge, it can fold out revealing needed information on you.

Step 4: with the information inside and the cover card in place, laminate it. This can be done a number of ways. I took mine to Office Depot and had it laminated in a business card holder with a hole in the top so that it can be put on my ARES ID lanyard. But remember, you can make as many as you think you need and put them anywhere. In your wallet, in your car, on your refrigerator at home. Just anywhere you want.
